If you have received an Invitation to Consider Eligibility for the Levy letter and wish to certify:
Certification form (click here) for new Guernsey and Jersey Registered providers who have received an invitation to consider eligibility for the levy letter.
If you have received a levy notice and wish to certify:
The certification form (click here) should be completed by registered providers that have been issued with a levy notice and wish to certify that they are entitled to zero-rating after having due regard to the Guernsey or Jersey Levy Scheme and the explanatory notes above.
If you wish to certify in respect of some, not all, areas of business, click here
If you are no longer eligible for zero-rating and wish to certify:
The certification form (click here) should be completed by registered providers that have previously certified as eligible for zero-rating and are no longer eligible.
